PRIVATE MUSIC LESSON INSTRUCTION CONTRACT

1. _________________________, will receive a 30, 45, or 60 minute lesson once a week at a cost of ________ per week. Payment for lessons are to be made once a month by the date of the first lesson. Payments can be made in person at the front desk by cash, credit, or debit, as well as personal cheque.
2. There is a once a year fee covering books, CDs, Binders, printed music, lesson plan sheets, and special student events. This fee for the 2011/2012 year is $20 and needs to be paid during the 1st payment of the fall semester for each student.

3. The student will receive an invoice (which outlines dates of paid lessons, credit for missed lessons, and upcoming monthly fees) by the 2nd lesson of the month.
4. If the payment is not received by the fifteenth of the month, a late fee of $5.00 per student will be assessed. This late fee will be added to the next month’s bill if it is not included with the payment. You will receive both a phone call and email reminder from Jen before the fifteenth.
5. Every effort will be made to see the student each week. If the teacher must cancel for any reason and is unable to reschedule, a credit of the lesson missed will be given to the next month’s payment. The student can cancel a lesson up to the day before a scheduled meeting and still receive credit. If the student needs to cancel a lesson, inform Jen or your teacher (if possible) of the conflict as soon as possible. The first option is to reschedule the lesson. Any cancellations received on the day of the lesson except for sickness or weather will be rescheduled if possible, but if missed may not receive credit. This includes a “no show” lesson.
6. The most vital element in learning a musical instrument is Consistent Practice. The minimum goal of practice time is 30 minutes 4-5 days a week. Occasionally, there will be weeks of irregular practice due to illness, vacations, etc., but the student is expected to maintain this minimum (or more) faithfully.
7. Most lessons are scheduled back to back, therefore, it is of utmost importance that the student be ON TIME for the lesson.
8. It is VERY DIFFICULT to work without music. Be sure to bring the assigned music to every lesson. Also, the student should bring any other materials requested by the teacher. All music can be ordered through Jen, and all expected materials will be provided for each student – unless teachers have asked for additional items – and these could be purchased through the school.
By signing this contract, the student are committing to lessons on a semester basis. It other words, if the student terminates lessons, it should occur at the end of the month, and if possible at the end of a semester (December, April, June). Any money that has been paid in advance will not be refunded if the student chooses to terminate lessons in the middle of the month. For beneficial experience and overall musicianship success will be seen week to week and month to month if desire, effort, and practice are maintained.
